Wondering if any one can help....
I'm working on my DOR and I have a couple of questions:
I'm confused on the Birth Registration number. In GA, we have two numbers: State File No. XXXXX and below it a Custodian's No. XXXX. Do I state both numbers or just the State File number? My belief is that the Custodian number was generated at the hospital then the State file number was generated by them when they received the certificate.
My second question is an address for the hospital necessary if it is nowhere on the COLB?

Just use the state number and the LF number is the local File #
