Started working on DOR but have some questions as I am currently married to my 2nd husband and the form notes do not specify:
1) Is the 1st page supposed to be blank?
2) Requested by and mail to should be :First Name-Middle Name; .Maiden Name-Current Married Name: correct?
3) The 3 variations in the opening sentence should only include maiden last name correct, not any married last name?
4) When it refers to "of the House of ____:", do I only input my maiden name or maiden-current married name or maiden-previous married-current married?
5) Autograph in red? using Parse syntax? on right side of page?
